Patient forms
New patients will receive a confirmation email with a link to fill out your medical history and consent forms online. If you are unable to do this electronically, please have these forms (see below) printed and completed prior to your scheduled time. Otherwise, please arrive 15 minutes prior to your scheduled appointment to fill out the forms.
does your insurance cover acupuncture?
Some policies do offer acupuncture coverage and we’d be happy to verify your benefits for you.
FSA and HSA payment is also accepted.
